The emerging evidence suggests that older people, men, and people from Black, Asian and Minority Ethnic (BAME) communities seem to be at greater risk from Covid-19. The causes of these increased risk factors are not yet fully understood, and further research is taking place right now. However, we believe that it is important for us to respond quickly to what the evidence is telling us which is why we would like you to complete this self-assessment.
This self-assessment will help us identify your level of risk to ensure you are not put into an area with an inappropriate level of risk. It will also enable us to advise Trusts on any adjustments which may be necessary given your self-assessed level of risk. All the information on the form will be treated in strict confidence. It is important that you give a true and full account. The medical details you disclose on this form remain confidential to the Occupational Health service of NHSProfessionals and will not be disclosed to anyone else without your explicit consent.
